I provide therapy for adults. My normal session is 45 to 50 minutes. Evening sessions available.

I accept the following insurance plans:

• Medicare Advantage (through Aetna, Anthem, Blue Cross Blue Shield of Arizona, Cigna, United HealthCare)

• United HealthCare

If I do not accept your insurance, or your insurance is out of network, my fee per session is $130.

I accept payment via cash, check,Visa, Mastercard and PayPal.

Upon request I will provide a monthly statement of fees paid.

I require 24 hour notice of cancellation except in cases of emergency. There will a $88 fee if notice is not given.
